About the Challenge

Student Wellness invites you to participate in a research study: the Healthy Hawk Challenge. If you agree to be in the study, you will be asked to complete the confidential online wellness survey that collects information to assess health behaviors in all areas of wellness – fitness, nutrition, sexual health, substances, stress, etc.

Students who participate in the study will receive $5 for their time: U.S. citizens will get a $5 electronic Amazon gift card; students who are not U.S. citizens will receive a $5 check. Some students may be invited to participate in other programs or services based upon their survey results and can receive an additional compensation.

Participation is completely voluntary. After completing the survey you will receive personalized health information, identifying areas where you are doing well and areas you could improve, and access to a wide range of tools to help you meet your health and wellness goals.

Who and What

What is the purpose of the Healthy Hawk Challenge?

The purpose of the Healthy Hawk Challenge is to assess and address health risks for students at the University of Iowa through the use of a wellness survey.

What is Student Wellness?

Student Wellness offers a variety of free wellness services for students, including 1-on-1 consultations (stress, fitness, nutrition, tobacco cessation, alcohol/drugs, sleep), online and group programs, and events. Student Wellness has two locations: Westlawn (inside Student Health) and the Wellness Services area in the Campus Recreation and Wellness Center. The Survey

Will my results be kept confidential and anonymous?

Yes, the anonymity and confidentiality of your results are protected by the Health Insurance Portability and Accountability Act (HIPAA). All survey procedures are reviewed by the Institutional Review Board (IRB).

How long will the survey be open?

The survey will be closed by the end of the academic year. Once it is closed, you will no longer be able to access the survey.

Can I finish the survey later?

Yes. Your completed answers will be saved and you'll be able to finish the survey at any time.

Can I change my answers after I view my results?

Once you submit your survey and you view your results, you will not be able to go back and change your answers. We encourage students to view their survey answers before submitting to assure accurate results.

Can I take the survey multiple times?

No, the survey can only be completed once per academic year by a given student.

Is the survey database secure/safe from hackers?

Yes, the survey database is secure. The University of Iowa Information Technology Services (ITS) have taken a number of steps to safeguard the survey site from intrusion and perform regular monitoring to assure quality security.

Can I invite my friends to take the survey?

Only eligible sophomore and junior students (as classified by the University of Iowa Registrar's Office) may take the Healthy Hawk Challenge survey at this time. They will receive invites via email if they qualify.

The Rewards

What can I use my $5 gift card for?

You can use your $5 gift card towards the purchase of any items(s) available at Amazon.

How long will it take after completing the survey to receive my gift card (or check if I'm not a U.S. citizen)?

With a successful submission of the survey, a code will be emailed to you to redeem your Amazon gift card within 10 days. If you are a non-US citizen, you should receive your check within two to four weeks from the date of your survey submission.

What if my gift card or check is stolen or gets lost in the mail?

We are not responsible for lost or stolen gift cards and checks.
